I've submitted about 100 different PQDR's for the mechanical lift pumps, they always tend to leak from the top seal, or come in broken. This is the last one that came in broken.

You will need an account. From here you can submit PQDR's and SDR's. Make sure you check with your local MMO SOP. Some units require all PQDR's to go through their office. Regardless, having an account allows you to see trends in equipment, and many times the answer to your problem is in the PQDR's someone else already submitted.
